The Business Center will send an email before each semester starts with two Database Templates requesting the information for the returning faculty and student appointments.

Enter the information required in the corresponding template and return back to the Business Center by the specified due date.

Templates:

1. 1. 1. Academic (Returning Faculty-Part-Time, Lecturers/Adjunct Faculty & Ph.D. TA, Ph.D. AI, MTA)
      2. Research Template (Faculty, Ph. D. RA, MRA, URA paid with grants)

    

   1. 1. 1. Academic template, include the following information:
            • Employee’s Name
            • Employee’s ID
            • Position Name
            • Position Number
            • Academic Rate
            • Pay for Course or hours/week (1-3 credit hour = 8 hours)
            • Cost Center
            • Part of Term
            • Course Number
            • Course CRN
            • Notes for the Business Center

    

   1. 1. 2. Research template, include the following information:
            • Employee’s Name
            • Employee’s ID
            • Position Name
            • Position Number
            • Academic Rate
            • Appointment Percentage
            • Project ID
            • Start Date
            • End Date
            • PI’s Name

    

   If you have new faculty or faculty who have a break of service of 6 months or more, request the Criminal Background Check to the Business Center and include the offer letter.

   Do not include any hourly students through the Database. Those appointments will need to be submitted manually through the Business Center website.

   Semesters (Part of Term):
         Fall (September 1 – January 15)
         Spring (January 16 – May 31)
         Summer

   • SU1: June 1 – 30
   • SU2: July 1 – 31
   • Long Summer: June 1 – July 31

NOTE:

• MTAs can only work June through July during the summer depending on the courses they are assisting.
• MRAs can work June through August.

Please submit two files (Do not change the forms): If the appointments are not submitted in a timely manner and approved through the Database, they will need to be requested via the Business Center.
